臨床研究“設(shè)盲” (blinded)：與眼睛看不見(blind)有什么關(guān)系？ - 知乎首發(fā)于羊君醫(yī)學(xué)翻譯筆記切換模式寫文章登錄/注冊臨床研究“設(shè)盲” (blinded)：與眼睛看不見(blind)有什么關(guān)系？陳數(shù)羊csy為了讓非醫(yī)學(xué)小伙伴理解“設(shè)盲”（blinded）的意思，我們設(shè)定一個小例子。如果想研究 XX 精華液是不是真的有美白效果，我們將受試者分為兩組。一組用精華液，另一組用跟精華液看起來一樣的白水。為了避免心理作用，兩組受試者不知道自己用的是哪個，這就是“單盲”。如果研究者自己也不知道兩組受試者具體用什么，那就是“雙盲”。 那么，“設(shè)盲” （blinded）與眼睛看不見（blind）到底有什么關(guān)系呢？最早的“設(shè)盲”試驗跟美國政治家富蘭克林（Benjamin Franklin）有關(guān)。18世紀(jì)70年代，德國醫(yī)生麥斯梅爾（Franz Mesmer）掌握一種據(jù)說可以治病的“催眠術(shù)”（Mesmerism）。（Jensen, M. B., et al. 2007）他認(rèn)為，天地萬物間存在一種難以察覺的流體，叫作“動物磁力”(animal magnetism)。如果身體中的這種磁力失衡，人就會生病。醫(yī)生可以借助雙手將宇宙間的磁力引向人體，磁力恢復(fù)平衡，疾病就治愈了。麥斯梅爾表示，這種“催眠術(shù)”可以治療很多疾病，包括頭痛和癲癇。這種神奇的“催眠術(shù)”受到民眾追捧，但也存在爭議。因此，富蘭克林等人被任命調(diào)查這種方法是否真的有治病效果。受試者被蒙上（blindfolded）眼睛參與試驗。試驗結(jié)果證明，這種所謂的“催眠術(shù)”治病法只是心理安慰作用。這就是“設(shè)盲”的由來，雖然早在 18 世紀(jì)就出現(xiàn)了，但直到 20 世紀(jì) 50 年代才開始廣泛使用。從歷史溯源可以看出，“設(shè)盲”就是“使看不見”。如果我們從單詞構(gòu)詞來看，也很有意思。blinded 這個詞是 “blind”+ “ed”。我們高中時就學(xué)過，大多數(shù)情況下，一個動詞的ed 形式（過去分詞）表示被動。所以，ed 形式的單詞大多都帶有 “人的行為引起”的含義。blind就是“看不見的”自然狀態(tài)，而blinded是“使看不見”，是一種延伸義。類似的區(qū)別，再舉1組例子：repeat vs. repeated相對來說，兩者作為形容詞時，repeat就偏向事物本身的重復(fù)，而repeated就帶有人的作用。在日常生活中，很多重復(fù)可能是人為的，所以repeated用得多一些。但在講究客觀的醫(yī)學(xué)中，repeat頻率相對也很高。我們在使用時，也可以區(qū)分一下語境。例句：1. A repeat TTE reportedly showed a symmetric left ventricular wall thickness of 20 mm.2. The sample-size justification and the methods for repeated random sampling of households that were used in our previous survey have been published.在臨床研究語境中，另一個詞 masked 也有必要提一下，因為 masked 也可以表示“設(shè)盲”。關(guān)于blinded（blinding）和masked（masking）這兩個詞誰更好，《英國醫(yī)學(xué)雜志》（The BMJ）曾有兩篇文章“針鋒相對”地發(fā)表了各自的看法，也是很有意思。Morris D 等人（2007）表示，masking更好，因為 blinding 日常生活中使用較多，就是“眼睛失明”的意思。在臨床研究語境下使用，容易讓患者產(chǎn)生誤解和恐懼。而 Kenneth F Schulz 等作者（2007）認(rèn)為，在臨床研究中，blinding 的使用已經(jīng)有 200 多年的歷史，早就深入人心，不宜更換； 在 ICH 中，也主要使用 blinding。因此，還是 blinding 更好?？傮w來說，在臨床研究中，blinded/blinding 使用頻率更高；而 masked/masking 更偏向在眼科中使用。參考資料：1. 臨床試驗研究中的盲法知識知多少？ - 知乎首發(fā)于臨床試驗資訊與技能交流切換模式寫文章登錄/注冊臨床試驗研究中的盲法知識知多少？臨床試驗達(dá)人隨機(jī)對照臨床試驗研究是臨床研究中具有較高驗證能力的研究設(shè)計類型。為了避免研究者主觀因素對結(jié)果評定的影響，盡可能減少臨床試驗研究中的偏倚，從而提高臨床試驗合理性與科學(xué)性，盲法是一個較為有效的設(shè)計措施。1.什么是盲法？盲法（blinding或masking）：就是通過合理的科研設(shè)計，讓受試對象不知道自己究竟接受了治療措施還是對照措施；或者讓研究人員也不知道某個患者究竟在治療組還是對照組。 盲法的目的是盡可能消除受試對象或者研究人員主觀因素對科研結(jié)局的影響，從而減少信息偏倚。一般比較常見的盲性方案包括：（1）單盲法（single blind）：受試者不知道自己接受哪一種治療，但研究者知道，或治療研究者處于盲態(tài)；（2）雙盲法（double blind）：受試者和研究者均不知道受試者本人接受哪一種治療；（3）三盲法（triple blind）：受試者、治療研究者、檢查研究者都不知道受試者接受哪一種治療；2. 什么是單模擬和雙模擬？單模擬技術(shù)是指：申辦方僅需制備一種與研究藥物所對應(yīng)的安慰劑。在以安慰劑為對照的臨床試驗中，不存在兩組藥物在用法用量上的差異，安慰劑組可以完全按照試驗藥的用法用量。雙模擬是在以陽性藥為對照的臨床研試驗中，陽性對照藥是已上市或其他廠家的藥品，其外觀、用法、用量與試驗藥很可能完全不一致。在這樣的實驗中，如果要實現(xiàn)雙盲，需采用雙模擬技術(shù)，即由申辦者制備一個與試驗藥外觀相同的安慰劑，稱為試驗藥的安慰劑；再制備一個與對照藥外觀相同的安慰劑，稱為對照藥的安慰劑。試驗組的受試者服用試驗藥+對照藥的安慰劑；對照組的受試者則服用對照藥+試驗藥的安慰劑。因此每個入組病例所使用的藥物、藥物劑量、用藥次數(shù)等都是一樣的，這就保證了雙盲法的實施。3.什么是臨床試驗的編盲和揭盲?在對照試驗中，需要一方或多方不知道受試者使用的是對照藥品還是試驗藥品，就需要進(jìn)行編盲，編盲過程由生物統(tǒng)計學(xué)專業(yè)人員在計算機(jī)上使用統(tǒng)計軟件產(chǎn)生隨機(jī)數(shù)字表，隨機(jī)數(shù)產(chǎn)生時間應(yīng)接近于藥品編碼時間。隨機(jī)表必須有可以重新產(chǎn)生的能力，即當(dāng)產(chǎn)生隨機(jī)數(shù)的初值、分層決定后能使這組隨機(jī)數(shù)重新產(chǎn)生。在盲法試驗結(jié)束后，需要進(jìn)行試驗藥和對照藥的安全性療效比較，這時候就需要知道受試者具體使用的是哪個組別的，這個知道的過程就是揭盲，一般進(jìn)行2次揭盲，一次揭盲是進(jìn)行組別揭盲，如一個含有兩個組別的試驗，首次揭盲會知道A組和B組，但是不知道A組是試驗藥還是對照藥，在此基礎(chǔ)上進(jìn)行統(tǒng)計分析，明確A組與B組的區(qū)別，再進(jìn)行二次揭盲，此次揭盲會揭曉到底哪組為試驗組，哪組為對照組，進(jìn)一步分析臨床試驗結(jié)果。4.影響盲法質(zhì)量的因素有哪些？在藥物臨床試驗中為了實施盲法，我們需要在藥品準(zhǔn)備時下很多功夫，需要利用安慰劑、雙模擬技術(shù)、膠囊技術(shù)等使得藥物本身的特性不會破壞盲法。例如中醫(yī)藥中，很多藥物有特殊的氣味和顏色，因此在隨機(jī)化臨床試驗中，在制作模擬劑的時候必須在各方面與盡可能與治療藥物接近，因為不合格的模擬劑會增加破盲風(fēng)險。接受試驗藥物的患者可能會因為病情的改善/惡化或一些不良反應(yīng)的出現(xiàn)，從而猜測自己所接受的藥物治療類型，從而破壞盲法。另外，監(jiān)查員需要在整個試驗過程中督促相關(guān)人員不能有意無意泄露盲底。微信關(guān)注公眾號：臨床試驗達(dá)人，更多臨床試驗資訊和知臨床試驗知技能等你解鎖~~發(fā)布于 2021-02-23 16:49臨床試驗藥物臨床研究?贊同 22??1 條評論?分享?喜歡?收藏?申請轉(zhuǎn)載?文章被以下專欄收錄臨床試驗資訊與技能交流廣交志同道合朋友，共解臨床試
